they have a disease, get some treatment as soon as possible
Oscars do that when they are deficient in a certain nutrients. Are you feeding your koi properly?

Have you done a water change lately? If so, did you forget to add your dechlorinator? If not, do a 20% change.
What are your ammonia/nitrite/nitrate/pH readings?
If it’s twitching like it’s trying to shake something off than it could be a parasite. This is called flashing and could be treated with a dosage of PARAGUARD from Jungle Labs.
I believe it could be due to whitespot or some other parasitic infestation.
