have you noticed if the scales on the fish are raised at all? if so it could be that he has an internal infection that’s lead to the fatal dropsy.
if there are no raised scales, it could be an internal infection or constipation thats lead to an internal infection. try feeding your koi some fresh veges, cooked deshelled peas are good for the digestion and keep a close eye on him. i’m not so clued up on medicating pond fish.
I think you may be expecting a ‘happy event’. If the fish is otherwise healthy she’s more than likely about to lay eggs. Hope that’s the case!
I don’t know sounds like an internal infection to me.

Its a great anti-biotic food. Internal infections must be treated with either an anti-biotic shot or fed with meds. Adding meds to the water will do nothing but destroy your system. If you can find someone to administer the shot I’d do that too. Internal infections are serious and threaten the fishes life. I would isolate it if there is anyway possible. Feed all pond mates the anti-biotic food as well but don’t give them the shot. Good luck my friend.
This could well be a pregnancy rather than something more serious, if the scales are not ‘pineconed’ the fish is as you say otherwise normal them you may have some little ones soon! Several links below covering all bases-hopefully it’s not anything serious.
Loads of people saying that it could be pregnant, but make sure it hasn’t got a swim-bladder disease which can also bloat koi. You can usually get some anti biotic you add to the water (make sure it safe for pregnant fish just incase though).
She could be full of eggs. Are other koi fish chasing her around? because this is the time of year when fish spon.
Either that or your fish might have a tumor. If its a tumor than really the only way to get rid of it is to operate on it, and remove it. But don’t do it yourself, you could kill it. But if the tumor is in its gut then there really isn’t much chance that it will live.

more information is required for a full diagnostic search, has the fish lost weight? how is the swimming behaviour? is the fish feeding?
